Madison Heights Youth Baseball Association carries a secondary accident insurance policy to cover all players, coaches and volunteers.
The insurance carrier is Sadler Insurance.
This policy pays any remaining balance after your insurance has made payment or in the case that you do not have insurance coverage.
Once your insurance carrier has completed its payments you can fill out the attached insurance form, have the MHYBA Insurance representative help you with the organizations part and then send it in to Sadler Insurance for full payment
